Does Laparoscopic Unilateral Ovarian Cystectomy Reduce the Ovarian Reserve in Early Postoperative Period?

Abstract
Objectives: The aim of this study was to evaluate the impact of laparoscopic unilateral ovarian cystectomy for benign ovarian disease on the ovarian reserve in early postoperative period.

Methods: We retrospectively reviewed the medical records and laboratory data of patients aged 18~45 years with regular menstruation before gynecologic surgery. Thirty-three patients were included in this study who received scheduled laparoscopic unilateral ovarian cystectomy in Inje University Haeundae Paik Hospital, and received serum anti-Mullerian hormone (AMH) tests preoperatively and 3 days after operation. The patients were divided into three groups according to the histologic diagnosis: Group 1 included thirteen patients who were diagnosed as ovarian endometriosis. Group 2 included nine patients who were diagnosed as mature cystic teratoma. Group 3 included eleven patients who were diagnosed as other benign cyst.

Results: Postoperative day 3 AMH levels were significantly decreased after unilateral ovarian cystectomy compared to preoperative AMH levels in all patients (3.28¡¾0.34 ng/ml vs 4.27¡¾0.41 ng/ml, P=0.000). In subgroup analysis, there were significant differences between pre- and postperative AMH levels in group 1 (3.99¡¾0.65 ng/ml vs 2.68¡¾0.51 ng/ml, P=0.001). Postoperative AMH levels were also lower than preoperative levels in group 2 (4.07¡¾0.88 ng/ml vs 3.03¡¾0.71 ng/ml, P=0.011), and in group 3 (4.78¡¾0.67 ng/ml vs 4.18¡¾0.53 ng/ml, P=0.033).

Conclusion: Our results suggest that laparoscopic unilateral ovarian cystectomy reduce the ovarian reserve in early postoperative period irrespective of histologic diagnosis.
